I think it was Amy. Because he was thinking about Amy that day, and not any of the other people whose death he's responsible for (or thinks he is; "what's dead should stay dead"—there's a reason John's in this poll); he was thinking about Jo when he saw the blonde bartender, I'm sure, and he's always thinking about Sam. Also because Osiris heard him talking about Amy and not about anyone else. But I'm delighted to hear arguments to the contrary.

Really, I agree that it's Amy. But I liked that they never explicitly told us who it would be because, at least the way it hit me, it leaves room to think about the myriad of people Dean might be most guilty about.

Maybe ... could it have been Mary? I'm sure that he still feels like he should have been able to save her from YED. I would almost also add Samuel to that, but I'm not really sure that Dean feels guilty about what happened to him.

I wonder if the writers will turn around and have it be her, and we'll finally get some sort of payoff for all of the build up we had last season about mothers (buildup that seemed to fizzle out to me, even if we did see the appearance of the mother of all monsters for all of five minutes).
