Attending Oregon State Ecampus (read: online) classes requires applying to Oregon State. Applying to Oregon State requires submitting an 'insight resume'.
ETA:
Insight Resume:So what's an insight resume?
* Understanding of you as a unique, contributing individual.
* Your accomplishments, perspectives, experiences, and talents.
* Your achievements within the context of your social and personal circumstances.
* Participation in activities that develop academic, intellectual, and leadership abilities.
ETA:
1. Leadership/Group Contributions: Describe examples of your leadership experience in which you significantly influenced others, helped resolve disputes, or contributed to group efforts over time. Consider responsibilities to initiatives taken in or out of school. And responses are limited to a hundred words. This'll take some thought. Also some kicking impostor syndrome in the ass, because my immediate response is 'I haven't done any of these' (apparently my hindbrain thinks fanworks are not creative).
2. Knowledge in a field/creativity: Describe any special interests and how you have developed knowledge in these areas. Give examples of your creativity- the ability to see alternatives; take diverse perspectives; come up with many, varied, or original ideas; or willingness to try new things.
3. Dealing with adversity: Describe the most significant challenge you have faced and the steps you have taken to address this challenge. Include whether you turned to anyone in facing the challenge, the role the person played, and what you learned about yourself.
4. Community service: Explain what you have done to make your community a better place to live. Give examples of specific projects in which you have been involved over time.
5. Handling systemic challenges: Describe your experiences facing or witnessing discrimination. Tell us how you responded and what you learned from these experiences and how they have prepared you to contribute to the OSU community.
6. Goals/task commitment: Articulate the goals you have established for yourself and your efforts to accomplish these. Give at least one specific example that demonstrates your work ethic/diligence.

Think of it as an "about me" paper. They want to know what your hobbies and interests are, what type of involvement you have in the community (volunteering, etc.), and your overall disposition. It's best to spend a little time thinking about this, and to be truthful, the best source of information on you is your friends and relatives - those who know you best. Put together a little interview sheet, say 5 or so questions about you like:
What do you feel are my greatest strengths?
What are my weaknesses or areas that need improvement?
Do you see me as a leader or a follower?
....
Pick 3 or 4 people to interview and collect their responses. Tell them you want them to be honest, and to keep it objective, give them a pre-stamped return envelope to make it anonymous. Collect their responses, merge them with yours, and compose your "Insight Resume".
